Peyronie’s disease is a condition caused by scar tissue (plaque) forming inside the penis, leading to curvature, pain, and sometimes erectile dysfunction. While more common in middle-aged men, it can affect men of all ages.

According to the Urology Care Foundation, Peyronie’s disease has two stages:

  • Acute phase (6–18 months): Plaque forms, pain and curvature increase.
  • Chronic phase: Plaque stabilizes, pain usually subsides, and curvature stops progressing.

What Causes It?

The most common cause is repeated micro-injury or trauma during sexual activity. Instead of normal healing, excess collagen builds up, forming hard, fibrous plaques. Risk factors include genetics, age, connective tissue disorders, diabetes, and hypertension.

  • Penile curvature or deformity
  • Pain during erection
  • Shortening or narrowing of the penis
  • Hard lump under the skin
  • Difficulty with sexual activity

Treatment Options

There is no one-size-fits-all cure, but several effective treatments can reduce pain and improve function:

  1. Oral Medications

Vitamin E, carnitine, tamoxifen, and pentoxifylline may help reduce inflammation and slow plaque progression during the early phase.

  1. Injections

Medications such as Verapamil, Interferon α-2B, and Collagenase (Xiaflex®) can be injected directly into the plaque to soften or dissolve scar tissue. Collagenase is FDA-approved and can reduce curvature by 30–35%.
